Complete your contractor details, download your answers, and send the CSV to your Habodel contact with the supporting documents.

Answers stay in this page only and are lost if you leave or reload it. Nothing is submitted or saved online. The downloaded CSV includes your bank and contact details; send it directly to your Habodel contact.

Fields marked * are required. For other questions that do not apply, enter “Not applicable” or leave them blank. Include currency and whether VAT is included when entering rates.

Download and send

A CSV contains your answers only. Send copies of your public liability, employers’ liability and professional indemnity insurance, where applicable, plus an invoice separately to your Habodel contact.
